Alendronate (Fosamax)
Alendronate (brand name Fosamax) is a bisphosphonate medication used to treat osteoporosis and other bone disorders. It works by slowing down bone loss, which helps increase bone density and reduce the risk of fractures.
How it works
Alendronate works by binding to hydroxyapatite crystals in the bone, where it then inhibits the action of osteoclasts—the cells responsible for breaking down bone tissue. By reducing this process of bone resorption, the body can build up bone density.
Used to treat:

Osteoporosis: A condition that causes bones to become weak and brittle.

Paget's disease of the bone: A chronic condition of the skeleton in which bones grow abnormally large and soft.

Hypercalcemia: In some cases, it can be used to treat high levels of calcium in the blood.
Dosage and administration
To ensure effectiveness and minimize side effects, alendronate must be taken with specific instructions:

Timing: Take the medication first thing in the morning with a full glass of plain water, at least 30 minutes before eating or drinking anything else.

Position: Remain in an upright position (sitting or standing) for at least 30 minutes after taking the medication. Do not lie down.

Food and supplements: Do not take other medications, food, or supplements (especially calcium and antacids) within 30 minutes of taking alendronate, as they can interfere with its absorption.
Side effects and risks
Common side effects include:

Bone is living tissue that is constantly being broken down and replaced. Osteoporosis occurs when the creation of new bone doesn't keep up with the loss of old bone.
Osteoporosis causes bones to become weak and brittle — so brittle that a fall or even mild stress such as bending over or coughing can cause a break. Osteoporosis-related breaks most commonly occur in the hip, wrist or spine.
